Privacy Policy
Last updated: August 3, 2026
1. Introduction
Mark T. Galli, DDS ("we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect through gallidds.com (the "Site"), how we use it, and the choices available to you, including rights available to California residents under the California Consumer Privacy Act (CCPA).
2. Information We Collect
When you submit our contact form, we collect the information you provide directly: first name, last name, email address, phone number, and any message you write about the appointment or treatment you are interested in. This Site does not include any patient portal, online medical records, or appointment booking system that collects health information directly — scheduling and clinical intake are handled by our office once you contact us.
3. How We Use Your Information
We use the information submitted through our contact form solely to respond to your inquiry, schedule or discuss appointments, and provide customer service. We do not use your information for automated marketing without your consent and do not build advertising profiles from it. Any protected health information you share with our office once you become a patient is handled separately under applicable HIPAA privacy practices, which are provided to you directly at our office.
4. No Sale of Personal Information
We do not sell, rent, or trade your personal information to third parties for monetary or other valuable consideration.
5. Your California Privacy Rights (CCPA)
If you are a California resident, you have the right to:
- Know what personal information we have collected about you and how it has been used and shared.
- Delete personal information we have collected from you, subject to certain exceptions.
- Opt out of the sale or sharing of personal information. As noted above, we do not sell personal information, so there is nothing to opt out of at this time.
- Non-discrimination for exercising any of your privacy rights.
To exercise any of these rights, contact us using the information in Section 7 below. We may need to verify your identity before fulfilling your request.
6. Data Retention
We retain contact form submissions only as long as reasonably necessary to respond to your inquiry and maintain our business records, after which they are deleted or anonymized. You may request earlier deletion at any time by contacting us directly. Records related to your care as a patient are retained separately in accordance with California dental recordkeeping requirements.
7. Children's Privacy
This Site is not directed at children and is not intended for use by anyone under 13 without the involvement of a parent or guardian. We do not knowingly collect personal information through the contact form from children under 13. If a parent or guardian believes a child has submitted personal information to us without consent, please contact us and we will delete it. If you are booking an appointment for a minor patient, please submit the inquiry as the minor's parent or legal guardian.
